ICUPE: Sample Class Selection Submission Form

If your net browser is forms-capable, you may use the forms below to select the sample classes you would prefer to attend at ICUPE. You will be able to attend three (3) sample classes: one on each day of the conference. Since each class will be given only once, we regret that we will be unable to guarantee everyone's choice of sample classes due to conflicts and oversubscription. Please submit your six (6) top choices in the order given. We will do our best to try and assign everyone either to sample classes of their choice or to similar classes.

The best way to fill out this form is to read the description of the sample classes, select the 6 you prefer by number, and then enter them in the appropriate spaces below. When you have the class you want displayed in each of the six boxes, press the SUBMIT button at the bottom of the page.
